Bent valve, weak adjuster, too much constant rpm, big cam with crappy springs.....

Only time I ever lost one was with the solid lifters I was trying and one lost it's setting and a follower fell off.

I think its bent valves . I am going to pull the heads and check also replace the head gaskets.
